9:00 a.m.-4:00 p.m. 7/13/2010 Athletic Physicals to be given July 29 Athletic Physicals will be available for all MISD students on Thursday, July 29th. Cost will be $25. Dr. Trev Dixon will give physicals from 9-11am in the Athletic Hall at the MHS gym. All incoming 7th, 9th & 11th grade athletes are required to receive a new physical. Also, any students that have been injured and required medical attention from a Doctor may need a new physical. Parents will need to either be present to fill out the form or fill out a form prior to sending their child. Use the link below to print the physical form.
