If you would like your child to get a Flu vaccine a consent form will need to be filled out and returned to the school. We have attached the consent forms or you can download the consent at www.pchdtx.org (Outreach Department). We also have consent forms available in the front office.
18 and younger PCHD will bill insurance. For those without insurance or underinsured there will be no cost. For adults PCHD can bill most commercial insurance or it is $20.
Elementary Students: Parents, if you wish to accompany your child we ask that you get to the school at the start of the clinic (8:00 a.m.) and bring your child’s consent with you.
